How can I submit my nomination?

Nominations can be submitted through the HQI website. Click here for the nomination form.

Who will be viewing my nomination and making the selection?

The nomination committee is comprised of stakeholders in Maryland and Virginia, HQI staff and members of our Patient & Family Advisory Council (PFAC).

What is the time frame for nominations?

Nominations will be accepted through Friday, August 16, 2019.

What is the difference between the nominator and the nominee?

The nominator is the person who submits a nomination for review. The nominee is the organization that has been submitted to be considered for the award.

When will winners be announced?

Maryland winners will be announced on Thursday, November 14, 2019 during an awards luncheon in Maryland. Virginia winners will announced on Tuesday, November 19, 2019 during an awards luncheon in Virginia.

Is supplementary documentation required?

Nominations must include the completed nomination form (link) and a one-page narrative.

Are nominations not selected as winners kept confidential?

Yes. Nominations not selected as winners will be kept confidential.

Who should I contact if I have questions?

Are entries allowed for more than one category?

Yes. Entries are allowed for more than one category where applicable.

Will I receive feedback regarding my submission even if I am not selected?

Yes. You will receive a letter notifying you that we have selected other winners and thanking you for your participation.

If selected, am I required to attend the awards ceremony in person?

You are not required to attend the awards ceremony in person. If you win you will be announced during the awards luncheon and notified after the event.

Am I able to edit my nomination once it has been submitted?

No. Please make sure that your nomination is completed in its entirety before it is submitted.

Will I find out if I have won prior to the awards ceremony?

No. All winners will be announced at the awards luncheons.

What are the judging criteria?

Nominations will be judged based on the use of evidence-based approaches to quality improvement that deliver better care, better population health and lower health care costs.
