Care Management Coordinator - Virginia

Primary Responsibilities:

HQI is a health quality consulting company that serves as the Medicare Quality Innovation Network – Quality Improvement Organization (QIN-QIO) for Maryland and Virginia. In addition, we are the Practice Transformation Network (PTN) for a mid-Atlantic region, including Virginia, Maryland, West Virginia and Washington D.C., the only QIN-QIO in the nation to hold both contracts.” Since 1984, we have been leading the way to better healthcare by working with providers, patients and partners to improve health quality across the region. Through the PTN, we help independent practices to maximize efficiencies, manage population health and improve care coordination. HQI has helped improve patient care and safety for thousands of people over the years.

We are currently seeking a Care Management Coordinator who will assist HQI team members and provide quality coaching to physician practices enrolled in our Practice Transformation Network. In this role, you will work with practices and our team members to develop evidenced-based care coordination processes to drive better patient outcomes. Areas of primary focus will include chronic care management, transitional care management and referral management as well as preventive care to include annual wellness visits. This position will drive successful practice and network performance through enhanced care coordination practices and collecting and assessing meaningful data to improve clinical performance and patient engagement and self-management.

Applicants must demonstrate the ability to work independently and be part of cross-functional teams, develop strong consultative relationships, manage multiple priorities, interpret data, and design and implement plans of action using improvement tools and evidence-based interventions (i.e., PDSA, LEAN, etc.). Physician environment experience in practice transformation/quality improvement, ambulatory care setting quality and efficiency metrics, population health and care management structures, data analytics and electronic health records preferred.


Current and valid license to practice as a Registered Nurse in the state of Virginia. Requires a BA/BS; 3–5 years of health care experience or any combination of education and experience, which would provide an equivalent background. Excellent oral and written communication skills, including experience conducting training and writing reports. Strong analytical, data management and PC skills. Ability to travel between healthcare provider locations within the state and/or region.
