Improvement Consultant-Diabetes - Maryland/Virginia

We are currently seeking an Improvement Consultant with diabetes education experience participating in one of the nation’s largest healthcare quality improvement programs. In this role, you will coordinate with healthcare providers and community organizations on topics that may include:
• Referral of patients with diabetes or pre-diabetes for diabetes self-management education
• Identifying and working with organizations that provide or have the potential to provide these educational services
• Providing diabetes self-management education
• Tracking clinical data on patients with diabetes
You will apply the latest quality improvement tools and techniques while being part of the region’s most experienced and respected healthcare quality improvement organization.
As a home office based team member located in Northern Virginia or Southern Maryland, applicants must demonstrate the ability to work independently and be part of cross-functional teams, develop strong consultative relationships, manage multiple priorities, interpret data, and design and implement plans of action using advanced improvement tools and techniques (i.e., Project Management, PDSA, LEAN, Six Sigma, etc.). This position requires: proven ability and experience building meaningful connections within communities and target populations, proven project management experience and a proven ability to develop and deliver comprehensive diabetes training programs. Clinical Quality Management experience and/or work with partner organizations that provide these services is a plus.
Bachelor’s degree in a healthcare field or related area. Master’s degree or the equivalent combination of education and/or experience preferred. Minimum of 3 to 5 years of experience in healthcare administration, public health or quality improvement. CPHQ or equivalent credentials and experience desirable. Excellent oral and written communication skills, including experience conducting training and writing reports. Ability to travel the state and/or region.
