Program Manager, Accountable Health Community -

Primary Responsibilities:

Be on the forefront of innovation, addressing the “upstream” factors that impact individual and community health. Your experience managing a care coordination program, public health initiative or clinical-community partnership can put you there as the Program Manager for a group of health care providers and community partners working together to link thousands of Richmond-area patients with unmet health-related social needs to community supports.

As Program Manager, you will report to the Vice President and manage day-to-day activities that include coordinating an Advisory Board of community support organizations, health care delivery sites and local government. You will ensure all partners are properly screening patients for unmet basic needs and providing those patients referrals for community resources, as well as access to a personal navigator to assist with referrals. Ensuring the project is on track and meeting goals will be among your key responsibilities. Familiarity with the social determinants of health will be valuable, along with the ability to lead a diverse group of participants in the creation of shared policies and the development of strategies for closing resource gaps.

While ambulatory or hospital experience is desirable, it is not required. We are looking for a sharp, efficient, hardworking professional with a proven record of success managing multi-stakeholder initiatives and top-notch interpersonal skills to plan work, lead people and deliver high quality results in a complex health care environment.

A Bachelor’s Degree in healthcare administration, public health, social work, business or a related area is required. A Master’s degree is beneficial, along with three (3) to five (5) years of relevant professional experience. Healthcare quality improvement and community organizing skills are a strong plus, as is familiarity with clinical processes, workflow redesign, use of data systems and federal health care grants. This position is Richmond-based.
