Project Coordinator, Social Determinants & Practice Transformation - Virginia

Primary Responsibilities:

Be a part of the action on two of the most pressing challenges in health care today: addressing the social determinants of health and preparing clinicians for new care delivery and payment models. Put your project management experience, strong communication skills and familiarity with health-related social needs or health care payment reform to work as a Project Coordinator on HQI’s Physician Services Team.

This position supports two high-profile initiatives funded by the Center for Medicare and Medicaid Innovation: the Accountable Health Community (AHC) and Practice Transformation Network (PTN). The AHC we lead is bringing Richmond-area health care and community service providers together to identify and address health-related social needs like food insecurity at the point of care for more than 270,000 local residents. Through our PTN, HQI is helping more than 1,300 primary care providers in four states improve clinical and operational processes to deliver better care at lower costs.

As Project Coordinator, you will maintain positive relationships with stakeholder organizations and develop additional ones; coordinate our Advisory Board; identify, create and manage informational resources and educational content for project participants; ensure timely, consistent communication to participants and stakeholders; track performance on goals and support improvement activity; and prepare reports and responses to questions from our funder.

A Bachelor’s degree in health care administration, public health, business or a related area is required. A Master’s degree is beneficial. Three to five years’ health-related operational experience or practice management background is required. Experience managing multi-stakeholder projects, excellent written and oral communication skills, strong research skills, and a proven track record of fostering collaborative professional relationships are a plus. This position is Richmond-based.
