RVA Community Cares Part Time Project Assistant - Virginia

Interested in Social Determinants of Health (SDoH)?
According to the CDC, SDoH are the complex, integrated, and overlapping social structures and economic systems that are responsible for many health inequities. These social structures and economic systems include the social environment, physical environment, health services, and structural and societal factors.
What is the RVA Community Cares Model?
Sometimes patients’ other needs (SDoH) can affect how well they are able to deal with their health issues. The RVA Community Cares Model will test whether systematically identifying and addressing Medicare and Medicaid beneficiaries’ health-related social needs through screening, referral, and community navigation services impacts total health care costs and reduces inpatient and outpatient utilization.
The RVA Community Cares program can connect patients with community service providers to help them with SDoH, such as:
- Living Situation – homelessness, unstable housing, unfit housing due to pests, mold, etc.
- Food Insecurity
- Transportation (even non-medical transportation)
- Utilities Assistance
- Personal Safety – domestic, sexual or other violence
How can I help?
RVA Community Cares is looking for a part time project assistant to conduct screenings to identify Medicare and Medicaid beneficiaries in need. The project assistant will have the opportunity to learn more about SDoH, using systematic screening tools to identify populations in need, and how navigators or Community Health Workers (CHWs) can be deployed to help patients effectively address their needs.
