Central Virginia Transitions Collaborative Inaugural May Day Meeting

​May Day is traditionally a day to celebrate spring’s new beginnings, so it was fitting that a new care transitions coalition met on May 1 to kick off an initiative to improve the well-being of patients in central Virginia. Participants from a wide range of patient care organizations in several counties gathered at the Martha Jefferson Outpatient Care Center.

The welcome address by Amy Black, DNP(c), MSN, RN, NEA-BC, vice president and chief nurse executive, introduced and thanked the other members of the planning team (Lorna Facteau, RN, DNSc, chief nursing officer, University of Virginia Medical Center; Marta Keane, chief executive officer, Joint Area Board on Aging; and Carla Thomas, MS, CTRS, CPHQ, director, care transitions, VHQC) and noted “today is about what we can do together.”
