Health Facilitator, AHC -
Be a part of the action on some of the most pressing challenges in health care today: addressing health equity and the social determinants of health. Put your knowledge of community resources, strong communication skills and familiarity with health-related social needs to work as a Health Facilitator with Health Quality Innovators. Locally known as RVA Community Cares, this high-profile initiative funded by the Center for Medicare and Medicaid Innovation, the Accountable Health Community (AHC), is bringing Richmond-area health care and community service providers together to identify and address health-related social needs at the point of care for more than 270,000 local residents.
Your role in this model is patient-focused, conducting screenings that identify Medicare and Medicaid beneficiaries in need, then helping them connect with community resources for assistance for a variety of needs, ranging from food access to housing. This position works as part of the team aiming to reduce health disparities.

Desired Job Experience:
A Bachelor’s degree in social work, public health, or a related area is required. Experience in case management, patient care or customer service is beneficial. Attention to detail and strong oral and written communication skills are a plus.
