Improvement Consultant – Physician Services – Maryland -

Primary Responsibilities:

We are currently seeking an Improvement Consultant who will work with physician to improve health literacy, reduce the prevalence of diabetes and hypertension in conjunction with several partners in Southern Maryland. In this role, you will promote the adoption of clinical protocols, implement training and technical assistance of decision support tools, promote care models and screening tools, assist practices on workflow design, and help practices use technology including telehealth for population health management and referrals. You will help providers adopt evidence-based literacy and linguistically appropriate patient education materials, as well as adopt culturally competent patient-centered communication. You will apply the latest quality improvement tools and techniques based on evidence-based interventions. You will be part of the region’s most experienced and respected healthcare quality improvement organization, as well as work with esteemed partners working to improve care coordination and clinical outcomes. 

Applicants must demonstrate the ability to work independently and be part of cross-functional teams, develop strong consultative relationships, manage multiple priorities with tight deadlines, interpret data, and design and implement plans of action using improvement tools and evidence-based interventions (i.e., PDSA, LEAN, etc.). Familiarity with physician practices, clinical quality reporting, quality improvement strategies, workflow redesign, and electronic health records highly desirable.

Desired Job Experience:

Bachelor’s degree in a healthcare field or related area. Master’s degree or the equivalent combination of education and/or experience preferred. Position is remote, but candidate will have the ability to travel between healthcare provider locations within Southern Maryland.
