Primary Responsibilities:

Health Quality Innovators is seeking a nurse with local relationships to improve care and outcomes for stroke patients in Galax, VA and the surrounding area.

The Post-Discharge Navigator Consultant will collaborate with the Stroke Coordinator of Twin County Regional Hospital, located in Galax, VA to provide post hospital discharge navigation services to patients recently experiencing a stroke or those at risk of having a stroke. Responsibilities will include:

  • Provide transitional and chronic care management services as appropriate to qualifying patients
  • Coordinate continuity of patient care with healthcare organizations and facilities including primary care and specialty care providers
  • Support patient self-management of disease and behavior modification interventions.
  • Assess and support treatment adherence including medication management and reconciliation
  • Provide education and counseling to patients, family, and caregiver to support self-management, independent living, and activities of daily living
  • Identify “high-risk” patients (the chronically ill and those with special health care needs) and collaborate with care team to reduce the risk of readmission
  • Conduct screenings and address social determinant of health needs and coordinate referrals as needs are identified
  • Educate patient and family/caregiver(s) about relevant community resources
  • Identify, engage, and communicate with appropriate community-based providers, partners, and resources as needed to meet the needs of target patient population
  • As needed, organize and conduct community awareness events about stroke and/or chronic conditions that can lead to stroke

Desired Job Experience:

  • Three to five years’ experience in providing direct nursing care and prior case management, nurse navigator, or discharge planning experience preferred
  • Current license to practice as a Registered Nurse (RN) in the Commonwealth of Virginia
  • Bachelor’s degree in nursing preferred
  • Demonstrates excellent active listening, written, verbal communication skills
  • Experience with Motivational Interviewing or other behavior change communication techniques
  • Demonstrates ability to exercise initiative, judgment, organization, problem-solving, and decision-making skills
