Centra Health Takes a Community Approach to Improve Care Transitions

Like many communities across the state, Lynchburg has focused on reducing unnecessary hospitalizations. Through a new program, Take Charge, patients are receiving the support and encouragement they need to most effectively manage chronic health conditions at home on an outpatient basis.

The initiative began in 2012 when Centra and the Central Virginia Area Agency on Aging (CVAAA) learned about the VHQC’s Care Transitions Project . The project is based on hospitals collaborating with a community-based agency and using an evidence-based model to improve readmissions. Although relatively new, Centra and CVAAA‟s Take Charge program is already having a positive impact in the community.
