Harrisonburg Home Health Agencies Unite to Reduce Readmissions

Across Virginia, many communities are struggling with high hospital readmission rates among Medicare patients discharged to home health. Currently, readmission rates for this population are higher than those discharged to home or to a skilled nursing facility.
In Harrisonburg, four local home health agencies have taken note and are collaborating with Sentara Rockingham Memorial Hospital (SRMH) to make improvements in transitions from hospital to home health, and ultimately decreasing readmissions, as part of VHQC’s Care Transitions Project.
