Health Quality Innovators of the Year: Collaboration
Today we are profiling the Virginia Department for Aging and Rehabilitative Services, the winner of our Health Quality of the Year Awards in the Collaboration category.
The Virginia Department for Aging and Rehabilitative Services (DARS) helps older adults maintain their independence through a variety of programs and services. In collaboration with HQI, the Virginia Department of Health (VDH) and Virginia’s 25 Area Agencies on Aging, DARS offers diabetes, chronic pain and chronic disease self-management workshops. DARS partnered with HQI to develop a paperwork flow, conducted technical visits and held monthly statewide calls to ensure fidelity of the program, confirm paperwork was being completed properly and data was being captured. The collaboration resulted in 950 self-management workshops empowering 7,200 people with diabetes or pre-diabetes to improve their health. As a result, the collaboration prevented 54,214 avoidable hospital admissions, 15,454 avoidable hospital readmissions and generated a cost savings of $745 million.« Return to the News
