Meet Our Health Quality Innovators of the Year: Patient-Centered Care

On Monday,  we announced the recipients of our inaugural Health Quality Innovators of the Year Awards. Today, we are highlighting Capital Integrative Health and Harrisonburg Community Health Center, our awardees for the patient-centered care category.

Maryland: Capital Integrative Health, Bethesda, MD

Capital Integrative Health takes a collaborative, holistic approach to patient-centered care. Their multi-disciplinary team seeks to treat the whole person, not just an isolated set of symptoms, by spending more time with new patients to get a better understanding of their health history.

Using an integrative primary care model, clinicians partner with their patients to understand how lifestyle, diet, stress and other factors influence health and wellness. Mental health, physical therapy, lab services, patient education and other services are all offered on-site.

Virginia: Harrisonburg Community Health Center, Harrisonburg, VA

Harrisonburg Community Health Center’s commitment to its refugee and immigrant population is the definition of patient-centered health care. This nonprofit health care organization makes interpreters available at all points of service including medical, dental, women’s health and patient education. The Center’s website and educational material are available in Spanish and Arabic. The staff also hosted a back to school drive for children in the community and provides medication reconciliation and durable medical equipment consultation and fitting at its on-site pharmacy.

Beyond its efforts to make diverse families feel at home, Harrisonburg Community Health Center is also improving the health of patients with chronic conditions. As a participant in the Heart of Virginia Healthcare Initiative, funded by the Agency for Healthcare Research and Quality, the center improved quality measures related to aspirin therapy and blood pressure control.
