Partnership Leads to Care Transitions Coalition

Unnecessary hospitalizations are a challenge for patients, families and providers across Virginia. A new coalition, led by VHQC, the Virginia Hospital & Healthcare Association (VHHA) and the Virginia Department for Aging and Rehabilitation Services (DARS), is working to identify effective strategies and build strong partnerships that will improve care transitions for patients across the Commonwealth.
Individuals representing more than 20 organizations attended the first Virginia Partners for Care Transitions (VPCT) meeting on June 25. Participants learned more about the current challenges to improving care coordination and interventions for improving care transitions. Several experts in geriatric medicine and care coordination shared their stories and experiences.
