Riverside Health System Strengthens Collaborative Efforts to Improve Patient Care

​Health systems across Virginia are taking new approaches to reducing hospital readmissions through improved care transitions. At Riverside Health System, providers are reworking their approach to guiding patients through transitions and reaping the benefits of a collaborative, community-based approach.

Riverside has been actively engaged in the Eastern Virginia Care Transitions Partnership (EVCTP) administered by Bay Aging since 2012. This initiative was recently awarded funding through the Community Care Transition Partnership (CCTP) program led by the Centers for Medicare & Medicaid Services. EVCTP is among two organizations in the state to receive CCTP funding.

In addition to actively supporting EVCTP, Riverside has examined the factors that lead to unnecessary returns to the hospitals and developed a strategy to improve care for all of its patients and families.
