Take Action to Improve Care Transitions
Join Health Quality Innovators (HQI) as we celebrate the first Annual National Care Transitions Awareness (NCTA) Day on April 16. The Centers for Medicare & Medicaid Services (CMS) is inviting organizations across the country to discuss the innovative approaches they are taking to improve care transitions. Please join us in recognizing NCTA by committing to at least one of the following steps:
- Listen – Talk to your patients and families about the importance of safe, effective care transitions. Invite them to share their suggestions for improving the process.
- Celebrate – Share your successes within your organization by discussing how you are partnering with others in your community to improve care transitions.
- Innovate – Test a new approach in your practice, hospital or nursing home. Strengthen communication using Circle Back, learn strategies from a successful practice, partner with a local pharmacy to implement the Blue Bag initiative or protect patients from sepsis by educating staff and caregivers on the signs and symptoms.
- Share – Email us to let HQI know about the steps you are taking to improve transitions for your patients.
